We are so excited to incorporate in-person and/or virtual dance choreography for special events. Each participant must complete and submit the registration and release waiver below to participate in our class. Disclaimer: It is your responsibility to perform these dance activities in an open area free from obstacles. Please go at your own pace. By the use of our dance services, you agree to waive all liability and hold harmless Reason-a-BOWS, LLC and Danz-a-BOWS for any injury to yourself, to others, and property damage. You agree to participate in our dance class at your own risk.
RELEASE AND WAIVER FOR USE OF PHOTOGRAPHIC AND/OR VIDEO LIKENESS:
I hereby grant Reason-a-BOWS, LLC and Danz-a-BOWS permission to use my likeness in a photograph and/or video in any and all forms and media for publication, advertising, trade, and any other lawful purposes, without payment or any other consideration from Reason-a-BOWS, LLC and Danz-a-BOWS. I understand and agree that these materials will become the property of Reason-a-BOWS and Danz-a-BOWS and will not be returned.
In addition, I hereby irrevocably authorize Reason-a-BOWS, LLC and Danz-a-BOWS to edit, alter, copy, exhibit, publish or distribute this photo and/or video for purposes of promoting Reason-a-BOWS, LLC and Danz-a-BOWS programs and/or and services for any lawful purpose.
Further, I hereby waive the right to inspect or approve the finished product, including any written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph/video of my likeness.
I hereby hold harmless, release and forever discharge Reason-a-BOWS, LLC and Danz-a-BOWS from any and all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
My signature below acknowledges that I agree to all of the terms and conditions, waivers, and releases as set forth herein, and that I have read this RELEASE AND WAIVER FOR USE OF PHOTOGRAPHIC AND/OR VIDEO LIKENESS before signing below, and I fully understand the contents, meaning, and impact thereof.
